If you have been notified of a privacy breach, you can contact our Office for guidance with respect to how to file a complaint. If you require additional information with respect to the details of the breach, and how the public body or custodian is responding to the breach, you should contact the public body or custodian that notified you.
To file a privacy complaint under the Access to Information and Protection of Privacy Act, 2015 (ATIPPA, 2015) please use the ATIPPA, 2015 privacy complaint form. To file a privacy complaint under the Personal Health Information Act (PHIA) please use the PHIA Privacy Complaint form.
